We have started a dental plan payment system which would help patients enjoy a lot of benefits after being a member of the plan. Your dentist recognizes that you deserve the best dental care and is working harder than ever to make sure you receive it. To demonstrate this commitment to quality, your dentist has started a dental plan system for his patients which ensures that they are committed to a set of exacting standards in the dental care and service they provide to you. This dental plan Programme aims to improve communication between you and your dentist. You will receive information about your dental care and dental treatments available and will be involved in any decisions made about the dental care of your teeth and gums.

Dental Plan covers the following:

▪ Routine and preventative care
▪ Check ups
▪ X-rays
▪ Necessary fillings
▪ Hygienist visits
▪ Preventative dental advice and therapy
▪ Any necessary routine extractions
▪ Major restorative care
▪ Periodontal treatment
▪ Crowns* & Bridges*
▪ Dentures*
▪ Inlays*
▪ Root fillings (excluding laboratory fees)
▪ Worldwide emergency cover
▪ 24 hour worldwide emergency Helpline
▪ Dental accident cover
▪ Temporary emergency dental treatment

Dental Plan does not cover the following:

▪ Laboratory fees/ prescriptions
▪ Orthodontics
▪ Dental implants
▪ Any excluded treatment (detailed in the contract you signed
with your dentist) Specialist treatment
▪ Cosmetic treatment
▪ A minimal initial 'out of hours' call out fee

Dental Plan Questions and Answers

Your dentist will arrange a start date for your Dental plan care contract. This date will be entered onto your contract and will always fall on the first day of the month. Your supplementary Insurance will commence from the start date for your contract, or Dental plan`s receipt of your registration form, whichever is later.

Under Dental plan Care, the emphasis is placed on preventative dental care. This means that your dentist will work towards stopping dental problems before they occur. This will help improve and maintain your dental health. Your Dental plan Care dentist will explain your particular care and treatment needs.

Your Dental plan Care dentist determines your fee category. This is based on your existing dental condition. Your dentist may have completed the 'Oral Check' section on your assessment form. If so, this will highlight the scoring system used as a basis.

Dental plan Care benefits the whole family. Where more than one member of a family (or payment group) is registered with any Denplan dentist at one practice, the following discounts apply: 2 group members - 5% 3 group members - 10% 4 or more group members - 15% . Please note, all payments must be collected using the same method of payment (Direct Debit) and all correspondence will be sent to the payer's address only. If you would like to benefit from these discounts, please contact your Dental plan Care dentist.

You can choose to pay your subscription monthly by Direct Debit, or annually in advance, either by Direct Debit, cheque or by credit/ debit card to Teeth & More. Annual payments are calculated on a calendar year basis.

The registration fee is a one off payment equal to your monthly subscription. This will debited from your bank account during the first month of your contract. It will cover Dental plan Care`s initial administration costs to arrange your Supplementary Insurance.

Yes. You can cancel your contract at any time by giving at least 21 days notice by telephone, or in writing, to your dentist. The contract with your dentist will then end on the last day of the month which next occur.
